Direct answer
Notifications show review events addressed to a human across teams and workspaces they can access. A mention names them in a comment. An assignment makes them responsible for review work. Unread count and read/clear state belong to that person.
Human UI actions
Instruct the human to open Notifications, select an item to visit its related artifact or comment, or use Mark as read when no navigation is needed. If clearing items, have the human review and confirm the action.
State semantics

| Clear | Removes the item from the notification list after confirmation. |
| Resolve review | Changes the review thread; this is a separate action. |
Read or clear never grants access, resolves a thread, deletes a comment, or changes another person's notifications.
Destination failures
A notification can outlive access to its destination. If workspace access was removed or the artifact is no longer reachable, treat the denial as a terminal failure: retrying cannot help until a human restores access. Return a privacy-safe denial that does not expose or infer private artifact, owner, or recipient details.
